To receive a quote you will need to provide your name, date of birth, whether you smoke or use nicotine products or have done so in the last 12 months and the type of policy and sum assured that you need.
When you come to apply you will need to disclose further information including your medical history, any family medical history, lifestyle information and possibly employment information - by providing this information accurately the insurer will be able to make the correct decision about your application. Failure to provide full and accurate information could result in your policy being invalid and a claim being declined.
