As it will be a discount rather than a new contract (a loyalty bonus discount), after the discount period (not necessarily 12 months) it will be at the standard rate.

Best bet is to ask the provider directly what the cost will be, and to specify the discount and terms.

Say i get a £25 12 month contract today.
in 11 months i take the renewal discount which is say £5 off.
then for 12 months it will be £20.
11 months after that if i dont decide to take the renewal offer and go for an upgrade phone.

will my line rental stay at £20 or will it go back upto the full price which is £25?
